The Full UK Name Change Process
First, decide exactly what your full new name will be, including the precise spelling. Write it down clearly before you start the application.
Next, complete the correct online form, adult or child, and enter all details carefully. Your new name and address will appear on the document exactly as you type them, so check everything before submitting.

Download and print your document. It will already contain the information you entered, with spaces left for dates and signatures. If you are using a Deed Poll, it is sensible to print several copies.
Authorise the document correctly. A Deed Poll must be signed and dated in front of two independent witnesses. A Statutory Declaration must be sworn before a solicitor, who will stamp it.
Once completed, use your document to update your records with the organisations you deal with. You will need it when updating things like your passport, driving licence, bank records, and other official accounts.
